Spironolactone
Drug Class Description :

Potassium-sparing diuretics (K+- sparing diuretics).

Generic Name :

Generic

Presentation :

Tablets, spironolactone 25mg , 50 mg , 100 mg . Available from APS, Ashbourne (Spirospare R ), Cox, Hillcross, Norton. Oral suspension, sugar-free, spironolactone 5 mg/5mL , 10 mg/ 5mL , 25 mg/5mL , and 50 mg/5mL . Available from Rosemont (special order).

Indications :

Congestive cardiac failure, hepatic cirrhosis with ascites and oedema, malignant ascites, nephrotic syndrome, treatment and diagnosis of primary aldosteronism.

Adult Dosage :

Congestive cardiac failure, 100mg daily increasing to 400mg daily if required. Maintenance dose, 75-200mg daily. Other indications, refer to manufacturer's prescribing information.

Child Dosage :

Congestive cardiac failure, initially, 3mg/kg daily in divided doses. divided doses.

Contra Indications :

Severe renal impairment. Hyperkalaemia, hyponatraemia. Addison's disease. Pregnancy and lactation.

Special Precautions :

Hepatic or renal impairment. Monitor fluid balance and electrolytes. Long term use in young patients. Elderly. Porphyria.

Interactions :

K+ supplements, K+-sparing diuretics, ACE-inhibitors, cardiac glycosides, carbenoxolone, NSAIDs.

Adverse Reactions :

Electrolyte and metabolic disturbances. Gastrointestinal upset, lethargy, ataxia, confusion, rash, gynaecomastia, deepening of voice, menstrual irregularities. Hepatotoxicity, osteomalacia and blood disorders reported.
