CLIVARINE
Drug Class Description :

Low molecular weight heparins (LMWHs).

Generic Name :

Reviparin sodium

Drug description :

Solution for subcutaneous injection in prefilled syringes.

Presentation :

Injection, reviparin sodium equivalent to 1,432 units of anti-Factor Xa activity per 0.25mL.

Indications :

Prevention of thrombosis and thromboembolism.

Adult Dosage :

0.25mL once daily by subcutaneous injection. First injection should be given two hours before surgery. Continue treatment until patient is fully mobile; usual duration of therapy, seven days.

Child Dosage :

Not recommended.

Contra Indications :

Must not be used in central nervous system surgery, lumbar puncture, spinal anaesthesia and peridural anaesthesia. Known hypersensitivity to reviparin sodium and/or heparin hypersensitivity, including allergic thrombocytopenias. allergic thrombocytopenias. Conditions associated with an elevated bleeding risk, such as: haemorrhagic diathesis; coagulation factor deficit; severe liver, kidney or pancreatic disease; severe thrombocytopenia. Disease suggestive of vascular lesion, such as: active peptic ulcer; hypertension; haemorrhagic stroke; cerebral artery aneurysm; retinopathies; vitreous body haemorrhage; endocarditis lenta. Imminent abortion. Suspected malignancy associated with elevated bleeding risk. Kidney stones. Chronic alcoholism. Pregnancy.

Special Precautions :

Not for intramuscular administration. Avoid intramuscular injection of other medicines during treatment. Switching between different brands of LMWH during treatment not recommended. Risk of hyperkalaemia; measure plasma potassium before starting treatment then monitor regularly. Lactation.

Interactions :

Drugs affecting blood coagulation (eg, aspirin, coumarin derivatives, dipyridamole, dextrans, cephalosporins, high- dose penicillin). NSAIDs (eg, phenylbutazone, indomethacin), sulphinpyrazone. Nitroglycerin infusion. Further interactions, for example, with antihistamines, cardiac glycosides, tetracyclines, should not be precluded. Caution with drugs that raise serum potassium.

Adverse Reactions :

Local injection site reactions. Dose dependent: raised incidence of bleeding, particularly from the skin, mucous membranes, wounds, gastrointestinal tract and urogenital tract. Occasionally, mild thrombocytopenia. Rarely, severe thrombocytopenia, hypersensitivity reactions, anaphylactic reactions, alopecia, hyperkalaemia. Osteoporosis cannot be excluded with long-term treatment.
