Rifamycins / isonicotinic acid derivatives (antituberculosis drugs).

'150': pink s-c tablets marked EI and CG. '300': orange s-c oblong tablets marked DH and CG.
'150' - rifampicin 150 mg , isoniazid 100 mg . '300' - rifampicin 300 mg , isoniazid 150 mg .
Tuberculosis, certain mycobacterial infections.
Continuous therapy, under 50 kg, 3 x '150' tablets once daily; over 50 kg, 2 x '300' tablets once daily. Intermittent therapy, under 50 kg, 3 x '150' tablets twice or three times weekly; over 50 kg, 2 x '300' tablets twice or three times weekly. All as a single dose 30 minutes before breakfast.
Not recommended.
Acute liver disease, history of drug-induced hepatitis, peripheral neuritis.
Liver impairment; monitor function. Porphyria. Epilepsy. Pregnancy, lactation. Elderly, malnourished or very young patients. Monitor blood count.
Rifampicin is a potent inducer of liver enzymes while isoniazid inhibits drug metabolism, dose adjustment of concomitant therapies is probable; Drugs affected include: oral anticoagulants, oral hypoglycaemics, digoxin, antiarrythmics, methadone, phenytoin, hexobarbitone, nortriptyline, benzodiazepines, oral contraceptives, theophylline, dapsone, chloramphenicol, ketoconazole, itraconazole, cyclosporin, azathioprine, b-blockers, calcium antagonists, enalapril, cimetidine.
Bioavailability of rifampicin may be affected by antacids, opiates, anticholinergics, ketoconazole, PAS preparations containing bentonite. Isoniazid inhibits the metabolism of certain drugs, dose adjustment may be necessary when given concomitantly. Drugs affected include: phenytoin, carbamazepine, primidone, valproate. Other reactions include disulfiram, halothane, alcohol, antacids.
Influenza-like symptoms, usually encountered during intermittent therapy, may be prelude to thrombocytopenia, purpura, haemolytic anaemia, dyspnoea, bronchospasm, shock and renal failure; consider switch to daily dosing. Discolouration of body fluids, skin reactions, disturbance of liver function, increase in liver enzymes, hepatitis, jaundice, flushing, itching, conjunctivitis, hypersensitivity reactions, anorexia, GI upset, erosive gastritis, pseudomembranous colititis, fatigue, dizziness, confusion, muscle weakness, visual disturbances, blood dyscrasias, menstrual disturbances, crisis in Addison's patients.
Peripheral neuropathy, optic neuritis, convulsions, psychoses, dizziness, headache, liver function disturbances, hepatitis, rash, SLE, rheumatic syndrome.
