Drug Class Description :

Alkaloids.

Generic Name :

Nicotine

Drug description :

Nicotinell: 10, 20 or 30 cm2 transdermal patches marked CG CWC, CG FEF or CG EME respectively. Nicotinell Gum: original or mint- flavoured chewing gum. Nicotinell Lozenges: sugar-free mint-flavoured lozenges.

Presentation :

Nicotinell: patches releasing nicotine 0.7mg/cm2 per 24 hours; 10, 20 and 30cm2 sizes available. Nicotinell Gum: Nicotine 2mg ; 4 mg . Nicotinell Patches and Nicotinell Gum are not prescribable on the NHS. Nicotinell Lozenges: nicotine 1mg .

Indications :

Nicotine replacement as an aid to smoking cessation. Nicotinell lozenges are suitable for people who smoke under 20 cigarettes a day.

Adult Dosage :

Nicotinell: Under 18 years, not recommended. Over 18 years, more than 20 cigarettes per day, initially 30 cm2 patch once daily; less than 20 cigarettes per day, initially 20 cm2 patch once daily. Reduce patch size at intervals of 3 to 4 weeks. Max. treatment period 3 months. Apply patch to non-hairy area of skin on trunk or upper arm.

Apply new patch every 24 hrs. Stop smoking completely. slowly for 30 mins. when there is an urge to smoke; max. 25 x 2 mg or 15 x 4 mg pieces daily. Stop smoking completely. Withdraw gradually after 3 months. Nicotinell Lozenges: suck one slowly every one to two hours until the taste becomes strong. The lozenge should then be rested between the gum and cheek.

When the taste has faded it should be sucked slowly so that it remains whole for about 30 minutes. Maximum daily dose, 25 lozenges.

Child Dosage :

Under 18 years, not recommended.

Contra Indications :

Acute MI, unstable angina, severe cardiac arrhythmias, recent stroke, skin disease. Pregnancy, lactation. Occasional smokers. Nicontinell lozenges are not recommended for people with a strong nicotine dependency and should not be used by non- smokers.

Special Precautions :

Hypertension, angina, cerebrovascular or peripheral vascular disease, heart failure, hyperthyroidism, diabetes, renal or hepatic impairment, peptic ulcer. Advise patients to stop smoking completely when starting treatment, and to dispose of used patches carefully.

Interactions :

Theophylline, warfarin, insulin.

Adverse Reactions :

Local skin reactions (patches), headache, dizziness, GI upset.
